Include the page address, the rule you think is wrong, and a link to the current official agency page. You can describe a calculation using invented wage amounts. Please do not send names, Social Security numbers, claim identifiers, medical records or employer documents.
For a claim question
Use the agency link and phone on your state page. Weekly Benefit cannot file a claim, access an award, change a determination or contact an insurer for you.
